Dear Parents and Students:

You are invited to attend our Parent/Player Kickoff Meeting on Thursday Feburary 23. The meeting will be held in the Waverly High School Cafeteria.

The lacrosse team is open to all students (male and female) 4th through 12th grade. In order to be eligible for the Women's or Men's team grades 9-12 the student must be attending Waverly, Grand Ledge or Lansing Catholic High Schools.

Steps Required to "Sign Up" For 2012 Season

Note:

All steps listed below must be completed by  Monday, March 12, 2012 for player to begin practice.

 

Required forms are avalible here and will also be available at the Parent / Player Meeting, on Mar 1, 2012. Completed form can be turned in during the meeting or to your coach

 
  1. Submit completed Player/Parent Information Form.
  2. Pay Player Fee (see the fee structure and detaild info. below).
  3. Read and sign the “Athletic Code of Conduct and Handbook” or have on file for current school year at the Athletic Office.
  4. Signoff form: “Athletic Code of Conduct Signoff”
  5. Have current physical on file at Athletic Office for this school year or get new one and submit through your coach.
  6. Submit completed  “Parent / Player Participation Waiver” form

Equipment Policy

Players are responsible for providing their own equipment.

 

All equipment is mandatory for a player to have before they are allowed to participate in practices and games.

 

Men/boys: gloves, elbow pads, shoulder pads, stick, athletic supporter, and black helmet. 

 

Women/girls: sticks, mouth protection, eye protection.

Player Fees

$180: Varsity/JV men and women (includes cost of 2 bus trips to away games)

$50: Middle School boys (4th-6th grade)
$100: Middle School boys (7th-8th grade)

$50: Middle School girls (4th-6th grade)
$100: Middle School girls (7th-8th grade)

Note:

Completed forms and physical listed above are due by
Monday, March 12, 2012.   

Players that have not submitted their completed forms or paid their fees will not be allowed to participate in practice.

Players who need special consideration regarding the payment of fees, should contact their coach.
