Dear Parents and Students:

If you are interested in playing lacrosse this spring (practice begins March 10,2008). We would like to invite you to our Parent(s) / Player Kickoff Meeting.

The meeting will be held in the Waverly High School Cafeteria Wednesday, Jan 16th. The lacrosse team is open to all students (male and female) 4th through 12th grade.

We hope to see you there!

Steps Required to "Sign Up" For 2008 Season

Note:

All steps listed below must be completed by  Monday, March 10, 2008 for player to begin practice.

 

Required forms are avalible here and will also be available at the Parent / Player Meeting, on Jan. 16, 2008. Completed form can be turned in during the meeting or mailed to:

  1. Submit completed Player/Parent Information Form.
  2. Pay Player Fee (see the fee structure and detaild info. below).
  3. Read and sign the “Athletic Code of Conduct and Handbook” or have on file for current school year at the Athletic Office.
  4. Have current physical on file at Athletic Office for this school year or get new one and submit through your coach.
  5. Submit completed  “Parent / Player Participation Waiver” form

Equipment Policy

Players are responsible for providing their own equipment.

 

All equipment is mandatory for a player to have before they are allowed to participate in practices and games.

 

Men/boys: gloves, elbow pads, shoulder pads, stick, athletic supporter, and black helmet. 

 

Women/girls: sticks, mouth protection, eye protection. See handout for list of approved sticks and eye protection

Player Fees

$150: Varsity/JV men and women
$50: Middle School boys (4th-6th grade)
$100: Middle School boys (7th-8th grade)
$75: Middle School girls (4th-8th grade)

The player fee, completed forms and physical listed above are due by Monday, March 10, 2008.   

Note: Players that have not submitted their completed forms or paid their fees will not be allowed to participate in practice.

Players who need special consideration regarding the payment of fees, should contact their coach.
